Proactive Care Providers (PCP) Ltd. is an independent registered company with a wealth of experience in Health and Social Care. We provide a Person Centred Service for adults who have Learning Disabilities, Autism and Challenging Behaviour. Our aim is to support each individual to achieve their full potential for life, maximise their access to the community for social and learning experiences, and to offer choices that impact positively on their lifestyles.

PCP have a range of professionally trained staff who are committed to delivering the highest possible level of care and support to our service users.

The professional backgrounds of the staff employed by PCP include; nursing, psychology, education, and medical (we have a weekly session, and 24 hour on call support from a consultant psychiatrist). This range of experience and expertise allows us to undertake full, needs led assessments, and to create bespoke care/support plans and activity schedules.

	Respect for the individual and their family are paramount; we believe that whenever possible family members should be an integral part of the assessment and care planning process. We aim to engage all stakeholders in the process of Person Centred Planning, and welcome health and social care professionals to take an active part in building the support structures necessary for the individual to reach their full potential.
	Social Inclusion is essential is at the centre of our philosophy; access to and use of community facilities will form a large part of an activity schedule. For those individuals who have concerns about spending time out of their home environment, carefully designed programmes will be produced to reduce sensitivity, promote social activity and community access.
	All support plans, Health Action Plans, and activity schedules will be designed with the service user if at all possible; and most definitely with their family and professional support staff. All decisions will be based on the right to privacy and dignity, the choice of the service user, the wishes of the family, and existing assessments and support plans. At PCP we believe in the value of Research and Evidence Based Care delivery, and also in the validity of Value Based Care. Service Users’ will have individual structures to meet their needs, not the needs of the service or care home, routines have no place at PCP. Meals will be offered when the individual requires; activities will be scheduled around the service user and not shift times; staff will be available as they are needed by the individual.
	All people have inalienable rights, and we will ensure that rights of each service user are upheld. We will aim to strengthen decision taking for the individual; providing them with all the support and information they need, and in a format which is suitable for them. Our benchmark for each service user is for them to be Happy and Healthy, and we will work hard to achieve this.
